What Information is Protected? The Privacy Rule protects all "individually identifiable health information" held or transmitted by a covered entity or its business associate, in any form or media, whether electronic, paper, or oral. The Privacy Rule calls this information "protected health information". Eastside Center for Family is committed to following these guidelines.

A covered entity is permitted, but not required, to use and disclose protected health information, without an individual’s authorization, for the following purposes or situations: (1) To the Individual (unless required for access or accounting of disclosures); (2) Treatment, Payment, and Health Care Operations; (3) Opportunity to Agree or Object; (4) Incident to an otherwise permitted use and disclosure; (5) Public Interest and Benefit Activities; and (6) Limited Data Set for the purposes of research, public health or health care operations. Covered entities may rely on professional ethics and best judgments in deciding which of these permissive uses and disclosures to make.

How does our site handle do not track signals?

We honor do not track signals and do not track or plant cookies, or use advertising when a Do Not Track (DNT) browser mechanism is in place.

COPPA (Children Online Privacy Protection Act)

When it comes to the collection of personal information from children under 13, the Children’s Online Privacy Protection Act (COPPA) puts parents in control. We do not specifically market to children under 13.

Fair Information Practices

In order to be in line with Fair Information Practices we will take the following responsive action, should a data breach occur: We will notify the users via email within 7 business days and via in-site notification within 7 business days.

We also agree to the Individual Redress Principle, which requires that individuals have a right to pursue legally enforceable rights against data collectors and processors who fail to adhere to the law. This principle requires not only that individuals have enforceable rights against data users, but also that individuals have recourse to courts or government agencies to investigate and/or prosecute non-compliance by data processors.

CAN SPAM Act

The CAN-SPAM Act is a law that sets the rules for commercial email, establishes requirements for commercial messages, gives recipients the right to have emails stopped from being sent to them, and spells out tough penalties for violations.

We collect your email address in order to respond to inquiries, and/or other requests or questions.

To be in accordance with CANSPAM we agree to the following:

  • NOT use false or misleading subjects or email addresses.
  • Identify the message as an advertisement in some reasonable way.
  • Include the physical address of our business or site headquarters.
  • Monitor third-party email marketing services for compliance, if one is used.
  • Honor opt-out/unsubscribe requests quickly.
  • Allow users to unsubscribe by using the link at the bottom of each email.

If at any time you would like to unsubscribe from receiving future emails, you can follow the instructions at the bottom of each email and we will promptly remove you from ALL correspondence.

Please bear in mind that a complaint is a very serious action and should be done so when you believe professional misconduct has taken place. It is important that we resolve difficulties together should a problem arise. Part of therapy/treatment is learning new methods to communicate effectively. Before making your decision to report a health care provider, keep in mind that many common complaints such as scheduling problems, personality conflicts, or disputes over bills or insurance are usually not within the Department or board’s legal authority to take action. Reports involving fees or insurance claims are only investigated if there appears to be fraud involved.

Patient Rights

  1. You have the right to be treated with dignity and respect.
  2. You have the right to be treated without regard to race, color, creed, national origin, religion, sex, sexual orientation, age, criminal background, political beliefs, or disability.
  3. You have the right to expect that information concerning your treatment will be confidential.
  4. You have the right to examine your file in the presence of a staff member. 72 hour’s notice is required for file examination requests.
  5. You have the right to know all the services provided by or to which we can refer you, and to request participation in or referral to those services.
  6. You have the right to know the rules, procedures, and policies, to be notified of any changes and to ask for clarification of any item you do not understand.
  7. Treatment is voluntary. You may terminate your treatment at any time.
  8. You have the right to make suggestions to improve the range or quality of services provided and are encouraged to do so.
  9. You have the right to privacy, to freedom of religion, to communicate with significant others in the event of an emergency and to never be subjected to physical abuse, corporal punishment, or other forms of abuse by agency staff members.

10.You have the right to have clinical contact with a same-gender counselor, same-gender groups, or individual counseling where such issues as domestic violence, sexual assault, anger management, eating disorders and self-esteem can be safely addressed.

  1. You have the right not to be discriminated against or deprived of any agency service by agency employees or other patients, due to actual or perceived HIV infection, or association with individuals who are infected or may be perceived to be infected.
  2. You have the right to request referrals to other agencies or services as may be necessary to address any unresolved problems listed on the treatment plan; for example, domestic violence, sexual assault, anger management, self-esteem, mental health, parenting, child development, eating disorders and prenatal care.
  3. You have the right to be treated without regard to any disability.
  4. You have the right to have a reasonable opportunity to practice the religion of your choice alone or in private as long as such practice does not infringe on the rights of other patients receiving treatment services. You have the right to refuse participation in any service that is or is perceived to be a religious practice.
  5. You have the right to receive a copy of the patient’s right statement at admission and in case of disciplinary discharge per WAC.
  6. You have the right to know that we will provide all services to patients without regard to race, color, creed, religion, national origin, sex, age, marital status, sexual orientation, the presence of physical, mental, or sensory handicap (hearing, speech, vision), mental illness, veteran’s status, epilepsy, learning disability, HIV/AIDS, arthritis, cerebral palsy, developmental disability or multiple sclerosis, criminal background or political beliefs.
